Objetivo: El objetivo de este trabajo es la descripción pormenorizada de síntomas no
motores en Enfermedad de Neurona Motora (ENM), principalmente en la esfera
cognitiva y conductual, el estudio de la sexualidad en los pacientes y valorar la
interrelación entre todos estos aspectos y su repercusión en la sobrecarga de los
cuidadores/as.
Pacientes y Método: Se incluyeron 70 pacientes con ENM atendidos en los 3
hospitales públicos de Sevilla y sus cuidadores principales. Además de una entrevista
detallada en la que se interrogaba por dolor y disautonomía, se le pasaron varias
escalas. Al paciente: Escala de valoración funcional en esclerosis lateral amiotrófica
(ALSFRSr), Exploración neuropsicológica con la batería Edinburgh Cognitive and
Behavioral ALS Screen” (ECAS), valoración de los trastornos del ánimo con la escala
Hospital, Ansiedad y Depresión (HAD) y cuestionarios de sexualidad. La sobrecarga de
los cuidadores se valoró con el cuestionario Zarit.
Resultados: El dolor es un síntoma frecuente en las ENM: el 48,6% de los pacientes
presenta dolor nociceptivo y el 12,9% dolor neuropático. Como síntomas
disautonómicos, en nuestro estudio solo tiene relevancia la incontinencia urinaria en el
15,7% de los casos. Encontramos que la alteración funcional a nivel motor está
relacionada con el deterioro cognitivo (p<0,001), la apatía (p=0,009), las conductas
repetitivas (p=0,037), la ansiedad (p=0,026) y la depresión (p=0,007), ya que dichas
alteraciones aparecen con más frecuencia en pacientes con mayor deterioro motor. En
cuanto a la sexualidad, alrededor del 50% de los pacientes se encuentra sexualmente
activo. La presencia de apatía (p=0,026), demencia frontotemporal (DFT) (p=0,013) o
depresión (p=0,021) reducen la actividad sexual de los pacientes, mientras que no
hemos encontrado relación entre la actividad sexual y la alteración motora, la
presencia de ansiedad o los otros trastornos de conducta. La alteración de la memoria
interfiere en las fases de excitación (p=0,021) y orgasmo (p=0,019) en los pacientes
sexualmente activos. La sobrecarga de los cuidadores empeora con la presencia en el
paciente de deterioro funcional motor (p=0,01), depresión (p=0,024), deterioro
cognitivo (p=0,01), trastornos de conducta (como desinhibición [p<0,001], apatía
[p=0,001], pérdida de empatía [p=0,011] y conductas repetitivas [p=0,009]) y DFT
(p=0,024).
Conclusión: La ENM, especialmente la ELA como su forma de presentación más
frecuente, es una enfermedad multisistémica en la que, a falta de un tratamiento
curativo, hay que prestar atención a todas sus facetas para poder mejorar la calidad de
vida de los pacientes y sus cuidadores. Es preciso incidir en la atención al dolor,
disautonomía, así como a los trastornos cognitivos, conductuales y del ánimo, ya que
se asocian al deterioro motor, agravando la situación del paciente. La apatía y la
depresión reducen la actividad sexual y, en los pacientes sexualmente activos, la
alteración de la memoria interfiere en las fases de excitación y orgasmo. El deterioro
del paciente tanto a nivel motor como cognitivo y emocional, son los aspectos que más
incrementan la sobrecarga de los cuidadores, por lo que sería aconsejable hacer un
abordaje conjunto paciente/cuidador de estas alteraciones.
