Abstract | OBJETIVO: El objetivo de esta tesis doctoral fue evaluar los resultados en salud de un modelo de atención al paciente pluripatológico (PP) basado en la atención compartida entre atención primaria y hospitalaria.
METODOLOGÍA: ... OBJETIVO: El objetivo de esta tesis doctoral fue evaluar los resultados en salud de un modelo de atención al paciente pluripatológico (PP) basado en la atención compartida entre atención primaria y hospitalaria. METODOLOGÍA: Tras seguir las recomendaciones del Proceso Asistencial Integrado de Atención al PP de la Junta de Andalucía de 2007 y la evidencia actual sobre los modelos de atención a la cronicidad, se caracterizó un modelo de atención multidisciplinar que incluía cuatro intervenciones del médico de medicina interna, dos del médico de atención primaria, dos de enfermería y dos del farmacéutico hospitalario ( modelo de intervención del estudio IMPACTO). Se realizó un estudio prospectivo, multicéntrico, cuasiexperimental, para la aplicación del modelo de intervención del estudio IMPACTO en los 12 meses de seguimiento a los PP incluidos en el estudio. Se comparó la mortalidad global de la cohorte en estudio con una cohorte histórica multicéntrica de PP ( cohorte PROFUND). Se evaluó, el número ingresos y días de hospitalización, la situación funcional, la calidad de vida relacionada con la salud (CVRS) antes y después de la intervención, así como el efecto de cada una de las intervenciones en la supervivencia. RESULTADOS: Se incluyeron en el estudio 420 PP, con una edad media de 77.30±8.90, 52.6% hombres. No se encontraron diferencias significativas en la mortalidad global (IMPACTO 35.5% vs PROFUND 37.2%, p=0.52); Tras la intervención, el número de ingresos y los días de hospitalización se redujeron de forma significativa ( 1.63 ± 1.29 versus 0.96 ± 1.76, p<0.001 y 15.83 ± 16.39 versus 8.43 ± 13.65, p<0.001) y se objetivó una mejora en el componente sumario mental de la CVRS ( 43.05 ± 12.39 vs 46.31 ± 12.20, p<0.001). La intervención realizada no redujo la pérdida de capacidad funcional en los 12 meses de seguimiento. Los pacientes que no recibieron la visita programada del internista de referencia a los 15 días de la inclusión ni los cursos de educación para pacientes y cuidadores por enfermería tuvieron un riesgo de morir mayor en los 12 meses de seguimiento ( HR: 1.70; 95% IC: 1.16-2.49; p=0.006 y HR: 2.84; 95% IC: 1.90-4.23; p<0.001 respectivamente). CONCLUSIONES: El modelo de intervención del estudio IMPACTO sugiere una reducción el número de ingresos y días de hospitalización así como una mejora en la CVRS de los PP. Las visitas médicas tras la inclusión por el médico internista y los cursos de educación para PP se asociaron con una mejora en la supervivencia. |
