Abstract | La espondilitis anquilosante (ea) es una artropatia seronegativa que aparece en un reducido numero de pacientes en nuestro medio a la cual le han sido objetivadas alteraciones de la inmunidad y presenta un cuadro especifico ... La espondilitis anquilosante (ea) es una artropatia seronegativa que aparece en un reducido numero de pacientes en nuestro medio a la cual le han sido objetivadas alteraciones de la inmunidad y presenta un cuadro especifico de patologia ocular.Nosotros nos hemos planteado el estudio de la patologia ocular de estos pacientes el objetivar que alteraciones inmunologicas presentan y por ultimo correlacionar esta patologia ocular con el perfil inmune obtenido. Para ello hemos contado con 40 pacientes diagnosticados de ea -siguiendo los criterios clinicos de new york- por el departamento de medicina interna del hospital universitario (hus) de la facultad de medicina de la universidad de sevilla. Estos pacientes han sido estudiados en el servicio de oftalmologia del hus en la busqueda de la patologia ocular realizandosele a 30 de ellos y a 28 controles sanos una determinacion de linfocitos t4 t8 cociente t4/t8 y celulasnk. Nuestro resultados ofrecen el siguiente perfil genetico e inmune para la ea: 87%de positividad para el hla-b27 un aumento significativo de los linfocitos t8 y de las celulas nk y un valor significativamente menor del cociente t4/t8. Se encontro patologia ocular en el 77% de ello siendo esta la siguiente por orden de importancia: queratoconjuntivitis seca (37%) uveitis aguda anterior (15%) catarata (10%) y queratoconjuntivitis alergica (8%). Los resultados con respecto a la patologia ocular sugieren: -no cabe esperar una mayor o menor alteracion de la inmunidad celular en pacientes con ea por presentar o no una queratoconjuntivitis seca salvo las propiamente debidas a la enfermedad sistemica. -la uveitis aguda anterior puede ser un indicador clinico del descenso de linfocitos t4 en pacientes con ea. |
