dc.description.abstract | OBJETIVOS: Conocer las diferencias radiológicas producidas a nivel
lumbopélvico por la aplicación de la maniobra de alargamiento del test de
Downing para el análisis de la movilidad de la articulación sacroiliaca.
HIPÓTESIS: El test de Downing en alargamiento produce cambios en
distintas mediciones sobre radiografía simple anteroposterior,
compatibles con una rotación anterior del iliaco homolateral sobre el
sacro, en aquellos sujetos en los que el miembro movilizado se alarga de
forma considerable respecto al contralateral, no observándose en
aquellos sujetos en los que dicho alargamiento es mínimo.
MATERIAL Y METODOS: Se tomó una muestra de 50 sujetos de ambos
sexos, con edades comprendidas entre 18 y 62 años, que conformaban
dos grupos: uno en el que tras la ejecución de la maniobra de
alargamiento del test de Downing, el alargamiento visualizado en el
miembro inferior fue inferior a 0,5 cms (Downing negativo), y otro grupo,
a los que se les visualizó un alargamiento igual o superior a 1,5 cms
(Downing positivo). Antes y después de la realización de dicha maniobra
se realizó a todos los participantes una placa radiológica anteroposterior
de la pelvis. Sobre dichas radiografías se midieron el diámetro vertical y
transversal del ilíaco, el diámetro vertical y transversal del agujero
obturador, la distancia de la cresta iliaca a la cabeza femoral, la distancia
L4 a la cresta ilíaca, y el ángulo de inclinación de la base sacra. En el
caso de los 3 últimos parámetros, dicho estudio se realizó exclusivamente
en el lado donde se efectuó el test de Downing. El evaluador de la prueba
radiológica permaneció cegado respecto al grupo al que pertenecía cada
uno de los sujetos evaluados.
RESULTADOS: Se observaron diferencias significativas (p<0,05) entre
ambos grupos en las modificaciones de varios de los diámetros medidos,
disminuyendo más en el grupo del Downing positivo del lado movilizado

el diámetro anteroposterior del iliaco y el ángulo de inclinación de la base
sacra y aumentando el diámetro lateral del iliaco, el diámetro vertical del
agujero obturador, la distancia de la cresta iliaca a la cabeza del fémur y
la distancia de L4 a la cresta iliaca. No hubo diferencias en el diámetro
transversal del agujero homolateral del lado movilizado ni en ninguna de
las mediciones contralaterales (p>0,05).
CONCLUSIONES: Pueden observarse cambios radiológicos tras la
aplicación de la fase de alargamiento del test de Downing para la
sacroiliaca cuando se produce un alargamiento del miembro homolateral
mayor de 1,5 cms. Dichos cambios son compatibles con una rotación
anterior de la sacroiliaca homolateral a la realización de la maniobra. | es |
dc.description.abstract | OBJECTIVES: To assess the radiological lumbopelvic differences after the
application of the lengthening phase of the Downing test, designed to
evaluate the motion ability in the sacroiliac joint.
HYPOTHESIS: The elongation test according to Downing produces
changes in different measurements on plain Antero-posterior X-Ray,
compatible with previous rotation of the iliac homolateral on the sacrum,
in those subjects in which the mobilized limb stretches considerably with
respect to the contralateral, changes will not be observed in those subjects
in which the elongation is minimal.
MATERIAL AND METHODS: Fifty subjects (both male and female), aged
between 18 and 62, were included. They belonged to one out of two
groups: one with a negative result to the lengthening phase of the Downing
test (the lengthening of the lower limb was smaller than 0,5 cms), and
other group with a positive result to the lengthening phase of the Downing
test (the lengthening of the lower limb was bigger than 1,5 cms). Before
and after that manoeuvre, an anteroposterior lumbopelvic radiography was
taken of every subject. On these radiographies, several parameters were
measured: vertical and transversal iliac diameters, vertical and transversal
obturator foramen diameters, iliac crest to femoral head distance, L4 to
iliac crest distance, and the sacral tilt angle. The three last parameters
were only measured ipsilateral to the Downing test. The evaluator of the
radiographic measurement was blinded to the result of the Downing test.
RESULTS: Significant differences were obtained between groups (p<0,05)
for the modifications in several of the measured parameters, with a higher
decrease in the positive Downing test in the mobilized side for the
anteroposterior iliac diameter and the sacral tilt angle, and a higher
increase for the transversal iliac diameter, the vertical obturator foramen
diameter, the iliac crest to femoral head distance and the L4 to iliac crest

distance. No differences were found for the ipsilateral transversal
obturator foramen diameter and for none of the contralateral
measurements (p>0,05).
CONCLUSIONS: Radiological changes can be observed after the
application of the lengthening phase of the Downing test for the sacroiliac
joint whe a lengthening higher than 1,5 cms in the ipsilateral lower limb is
achieved. Those changes are compatible with an anterior rotation of the
ipsilateral sacroiliac joint. | es |
