Abstract | Infliximab es un anticuerpo monoclonal quimérico contra el TNF-alfa, utilizado en diversas enfermedades con gran impacto en la calidad de vida de los pacientes, como Enfermedad de Crohn (EC), colitis ulcerosa (CU) y artritis ... Infliximab es un anticuerpo monoclonal quimérico contra el TNF-alfa, utilizado en diversas enfermedades con gran impacto en la calidad de vida de los pacientes, como Enfermedad de Crohn (EC), colitis ulcerosa (CU) y artritis reumatoide (AR). Al ser un fármaco biológico, producido mediante tecnología del ADN recombinante, tiene el riesgo inherente de producir inmunogenicidad; por tanto, un gran número de pacientes sufren pérdida de respuesta al tratamiento al desarrollar anticuerpos contra el fármaco. Ante esta situación, la monitorización personalizada, además de detectar el posible fallo terapéutico por la aparición de anticuerpos, añade un parámetro objetivo a los criterios clínicos de respuesta, adaptando la cantidad de fármaco a la respuesta del paciente, lo que conlleva una mayor eficacia coste-efectiva. El objetivo de este trabajo es analizar la concentración sérica mínima de IFX y los niveles de anticuerpos antiinfliximab (AcIFX) en pacientes diagnosticados de EC, CU y AR en el Área de Gestión Sanitaria Sur de Sevilla. Metodología: se realizó un estudio observacional y retrospectivo, en el que se incluyeron pacientes adultos diagnosticados de EC, CU y AR, en tratamiento con IFX en fase de mantenimiento. La técnica utilizada fue el ELISA puente, Promonitor ®. Las concentraciones consideradas óptimas fueron 3-7 mcg/mL en EC y CU y >2,5 mcg/mL en AR. Resultados: un 23-40% de muestras presentaron concentraciones séricas óptimas. Se detectaron AcIFX en un 13,3-17,4%. La utilización de FAMEs se asocia con mayores niveles de fármaco y menor presencia de anticuerpos (en terapia combinada AcIFX 7%, en monoterapia 10,4%). La utilización de otros biológicos previos a IFX se relaciona con más presencia de AcIFX y niveles séricos indetectables. Conclusión: la monitorización farmacocinética de fármacos biológicos es considerada una nueva herramienta objetiva de ayuda en la toma de decisiones farmacoterapéuticas, sin embargo existe aún mucha incertidumbre que hacen necesario continuar realizando estudios para optimizar su utilidad. |
