Abstract | La Hepatitis C es una enfermedad global provocada por el virus del mismo nombre. Afecta a
180 millones de personas aproximadamente. La gravedad de la enfermedad puede ir desde una
dolencia leve con remisión espontánea ... La Hepatitis C es una enfermedad global provocada por el virus del mismo nombre. Afecta a 180 millones de personas aproximadamente. La gravedad de la enfermedad puede ir desde una dolencia leve con remisión espontánea hasta una enfermedad mortal que, en estados tardíos, llega a provocar cirrosis y carcinoma hepático, pudiendo dar lugar al deceso del afectado. Este contexto ha catalizado el avance en el conocimiento del virus, principalmente de su maquinaria proteica compuesta por diez proteínas de dos tipos: tres proteínas estructurales, que se encargan de la formación de la partícula viral y siete no estructurales, relacionadas con el proceso de replicación entre las que destacan NS3/4A, NS5A y NS5B. Además, el conocimiento del desarrollo del ciclo de vida del virus en el hospedador ha permitido identificar las etapas claves, las estrategias que el virus sigue para evadir la respuesta del hospedador, como la generación de una red membranosa que protege los intermedios de replicación viral de la respuesta inmune. Estos avances han llevado al diseño de nuevos antivirales de acción directa que inhiben a ciertas proteínas no estructurales. Aprobados a partir de 2014, estos principios activos han permitido lograr la curación de la enfermedad en casi la totalidad de los casos. Sin embargo, los costes de los tratamientos con dichos antivirales de acción directa han estado en tela de juicio por su elevado coste. En este trabajo se discute la inclusión de los medicamentos usados en el tratamiento de la Hepatitis C, centrándose en el de mayor relevancia clínica e industrial, y se analizan las ventajas aportadas. Además, se presentan aproximaciones a sus estrategias de síntesis química y se trata de comprender el diseño de los fármacos líderes en el tratamiento de la Hepatitis C, así como las perspectivas futuras. |
