Desde la síntesis de la aspirina en 1899, los AINE han sido uno de los fármacos más prescritos en el mundo para el tratamiento de la fiebre, la analgesia y los estados inflamatorios como la artritis reumatoide. Es bien conocido, tanto a nivel del profesional sanitario como culturalmente por la sociedad, que este grupo de fármacos pueden causar daño gastrointestinal (GI), llevando a la aparición de inflamación, ulceración, sangrado y perforación. También se sabe de los riesgos cardiovasculares y renales que conlleva su uso prolongado en el tiempo. No son pocos los casos de hipersensibilidad o alergias a alguno de estos principios activos. Sin embargo, el uso generalizado de estos medicamentos se mantiene y de forma homogénea entre los casos de automedicación y de receta. En vista a los hechos, la Agencia Española de Medicamentos y Productos Sanitarios (AEMPS) elaboró en al año 2014 un documento con una serie de recomendaciones para el uso adecuado y seguro de los AINE. En el presente trabajo se evalúa el cumplimiento de estas recomendaciones en la prescripción a nivel hospitalario, concretamente del del Hospital Universitario Virgen del Rocío de Sevilla, a través de las historias clínicas de un total de 100 pacientes pertenecientes a diversas áreas hospitalarias dotadas de prescripción electrónica. El resultado obtenido es que los riesgos GI son bien conocidos y prevenidos con la administración de omeprazol pero de forma protocolaria, no existe estratificación individualizada del riesgo digestivo. Los riesgos cardiovasculares y renales tampoco son evaluados en cada paciente, encontrándose bastantes casos de contraindicaciones por insuficiencia renal (16%) e interacciones entre AINE y fármacos antiagregantes y anticoagulantes (52%). Si bien es cierto que el hospital es un entorno en el que los pacientes se encuentran controlados y diariamente se evalúa su situación clínica pudiendo detectarse rápidamente la aparición de cualquier efecto adverso, se puede concluir que las recomendaciones de la AEMPS no se cumplen en la mayoría de los casos. |
