Relación entre la periodontitis y la artritis reumatoide : revisión bibliográfica

Abstract

Introducción: La enfermedad periodontal es una de las dos enfermedades graves dentales que afectan a la población humana en el mundo con una alta tasa de prevalencia. Por otro lado, la artritis reumatoide es la artritis inflamatoria más común, que afecta del 0,5 al 1% de la población general en el mundo. El objetivo de este estudio es realizar una revisión bibliográfica de la literatura sobre la posible relación entre la periodontitis y la artritis reumatoide (AR). Metodología: Se llevó a cabo una búsqueda en la base de datos Pubmed desde el año 2012 hasta el presente. Se obtuvieron 270 artículos, de los cuales fueron 30 los que se ajustaron a nuestros criterios de inclusión y de exclusión. Resultados y conclusiones: Tras la revisión bibliográfica hemos destacados datos significativos sobre la relación de la periodontitis con la artritis reumatoide. Según una revisión reciente con metanálisis, el riesgo de periodontitis en la AR frente a los controles es de 1.13. Un largo número de estudios muestran el incremento de la frecuencia de periodontitis en pacientes con artritis comparados con individuos sin artritis. Ambas presentan una patogenia común: inflamación, destrucción ósea, del tejido blando y activación de la respuesta inmune. La evidencia científica afirma la relación de ambas enfermedades a través de los patógenos periodontales, que expresan PAD, por la reducción de los parámetros de la actividad de la artritis tras la aplicación del tratamiento periodontal y el hecho de presentar factores de riesgo comunes como el tabaco.

Introduction: Periodontitis is one of the two serious dental diseases that affects the human population in the world with a high prevalence rate. On the other hand, rheumatoid arthritis is the most common inflammatory arthritis, which affects from 0.5 to 1% of the general population in the world. The aim of this study is carrying out a review of the existing literature with regards to the possible link between periodontitis and rheumatoid arthritis. Methodology: We carried out a search on the database Pubmed from 2012 to the present. We obtained 270 articles, 30 of which did fit the inclusion and exclusion criteria. Results and conclusions: After the bibliographic review, we have highlighted the significant data about the connection between periodontitis and rheumatoid arthritis. According to a recent meta-analysis, the risk of periodontitis in RA versus controls is 1.13. A large number of studies show the increased frequency of periodontitis in patients with arthritis compared to individuals without arthritis. Both present a common pathogenesis: inflammation, bone and soft tissue destruction, and activation of the immune response. The scientific evidence affirms the relation between both diseases through the periodontal pathogens, which express PAD, by the reduction of the parameters of the activity of the arthritis after the application of the periodontal treatment and the fact of presenting common risk factors as the tobacco.
