La neuropatía óptica hereditaria de Leber (NOHL) se encuentra entre las neuropatías heredadas de forma materna más comunes y fue la primera enfermedad mitocondrial descrita, causando una pérdida de visión bilateral con mayor prevalencia en los varones. La enfermedad es resultado de mutaciones en el ADN mitocondrial que conducen a una disfunción en el orgánulo, afectando así a la cadena respiratoria y la fosforilación oxidativa. Principalmente se ven afectadas las células ganglionares de la retina, produciendo la manifestación característica de pérdida de la visión, aunque algunas mutaciones menos frecuentes también pueden afectar a otros tejidos y como consecuencia mostrar otras manifestaciones además de la visual. Aun así, existen otros factores que modulan la pérdida de visión y su penetrancia según el sexo y las familias, tales como el estrés oxidativo, factores genéticos, factores hormonales y otros factores medioambientales que se ha visto que guardan relación con la mayor probabilidad de que un paciente manifieste la enfermedad, así como que algunos puedan presentar mayor recuperación de la visión después de la aparición de la enfermedad. La mayor preocupación actual es la búsqueda de los factores determinantes que producen la pérdida de la visión, a pesar de conocerse que las mutaciones en el ADN mitocondrial son una condición necesaria para la misma. Se están investigando nuevas terapias y tratamientos para curar la enfermedad una vez que ha aparecido así como para evitar el comienzo de la pérdida de visión inicial. Actualmente, los tratamientos más utilizados intentan ayudar a los pacientes que han quedado con la visión reducida, sin haberse conseguido eficazmente aumentar su visión ni una recuperación parcial. Se están llevando a cabo nuevas líneas de investigación encaminadas a frenar el inicio de la enfermedad, porque parecen ser más efectivas que las dirigidas a recuperar la visión una vez ha aparecido la manifestación inicial |
