Abstract | La lesión del nervio laríngeo recurrente constituye la lesión más signi<cativa de la cirugía tiroidea, junto al hipoparatiroidismo, y

La lesión del nervio laríngeo recurrente constituye la lesión más signi<cativa de la cirugía tiroidea, junto al hipoparatiroidismo, y aunque menos evidente aunque ocasionalmente representativa, la lesión de la rama externa del nervio laríngeo superior. En los últimos años la neuromonitorización intraoperatoria ha sido ampliamente tratada en múltiples aspectos, para de<nir su efectividad, seguridad y connotaciones médico legales, habiéndose publicado en la literatura metanálisis con muestras heterogéneas o insu<cientes, y trabajos individualizados que adolecen de rigor metodológico, tamaño y selección muestral y criterios de randomización que pueden confundir las conclusiones de esas investigaciones individuales. La Neuromonitorización intraoperatoria unánimemente se le considera como el complemento ideal al gold standard de la identi<cación nerviosa intraoperatoria, que satisface el desafío técnico, pero no descarta la consideración <siopatológica de que la conservación anatómica de un nervio por identi<cación visual no equivale a su integridad funcional. Analizamos la Neuromonitorización intraoperatoria intermitente y su valoración objetiva actual en el campo quirúrgico, así como las ventajas que ofrece la Neuromonitorización continua para la detección de efectos adversos que pueden evitar tras su apreciación y cambios de estrategia quirúrgica el desencadenamiento de una lesión nerviosa. |
