reducir el riesgo de infecciones y el desarrollo ... Introducción: En los pacientes con Púrpura Trombocitopénica Idiopática (PTI), los tratamientos prolongados, con dosis elevadas de corticoides son frecuentes. Con el fin de reducir el riesgo de infecciones y el desarrollo de osteoporosis/osteopenia, dos efectos adversos importantes asociados a este tratamiento, se podrían utilizar tratamientos profilácticos. Objetivo: Evaluar la incidencia de efectos adversos asociados al uso de corticoides a largo plazo. Como objetivos secundario analizar la frecuencia de uso y la eficacia de la profilaxis infecciosa y profilaxis para la osteoporosis/osteopenia. Material y métodos: Estudio de cohortes, observacional y retrospectivo de pacientes con PTI, tratados con corticoides entre el 20 de febrero de 1972 y el 1 de junio de 2023. Los pacientes con diagnóstico PTI fueron obtenidos del registro interno del Servicio de hematología del HVR y la recopilación de datos se realizó utilizando los programas Dyraya® y Athos Prismas®. Se construyó una base de datos en Microsoft Excel®. Para el análisis estadístico de datos, se utilizó el software SPSS® v25. Se emplearon estadísticas descriptivas y de frecuencia y tablas de contingencia con el estadístico de Chi cuadrado. Resultados: Un total de 164 pacientes con PTI tratados con CO en el HVR cumplieron los criterios de inclusión. La tasa de respuesta completa al tratamiento (≥100.000 plaquetas) fue del 44,5%. En general, los pacientes con PTI presentan más de una infección, con diversas localizaciones. Las infecciones hospitalarias son escasas (2,4%, n=4). La incidencia de infecciones domiciliarias es sin embargo mayor (28.1%, n=46). Tan sólo el 25,4% de los pacientes con PTI que precisan CO a largo plazo reciben profilaxis antiinfecciosa. Un total de 6,7% desarrollaron osteoporosis/ osteopenia. Sólo un 39,9% recibieron profilaxis para la osteoporosis/osteopenia. Se observa una mayor tendencia al desarrollo de osteoporosis/osteopenia e infecciones en los pacientes con un mayor tiempo en tratamiento con CO. Conclusión: El riesgo de sufrir infecciones y osteoporosis/osteopenias en pacientes con PTI aumenta conforme aumenta la duración del tratamiento. Es necesaria la implementación de protocolos de manejo de profilaxis de infecciones y de osteoporosis y osteopenia en pacientes con PTI. |
