dc.description.abstract | Este proyecto surge de la idea que Arturo Moreno Arteaga, estudiante del Grado en Ingeniería de
la Salud, propone al observar la dificultad que el autor de esta memoria (Álvaro López Tornos)
encuentra para desarrollar ejercicios con barras en el gimnasio debido a sufrir Síndrome de Poland
desde su nacimiento. Tras ser compañeros de equipo de baloncesto durante varios años y amigos de
clase desde pequeños, Arturo conoce bien las limitaciones que Álvaro tiene, por lo que propone,
aprovechando la rama de la Ingeniería en la que se especializa, llevar a cabo el diseño de una prótesis
que sirva para aquellas actividades que considera más complejas. Es entonces cuando Álvaro expone
la idea de poner en práctica ambos un Trabajo de Fin de Grado conjunto, con temáticas distintas
pero con un nexo común, el diseño de una prótesis de mano parcial para agarre de barras cilíndricas
en gimnasio.
De esta forma, y observando la infinidad de ideas que a día de hoy se implementan en prótesis,
los alumnos deciden dividir sus trabajos en un desarrollo de diseño mecánico (Álvaro), y un aporte
de electrónica que dote de funcionalidad a la prótesis (Arturo), ambas temáticas acordes a lo estudiado en los respectivos grados de ambos alumnos. Así, ambos alumnos se han podido lucrar del
conocimiento de su respectivo compañero.
Actualmente el ámbito de las prótesis sigue un crecimiento exponencial, el desarrollo de nuevas
tecnologías, así como la variedad de amputaciones y malformaciones hace que la diversidad protésica en el mercado sea inmensa. Sin embargo, es ínfimo el número de estas que se centra en el
levantamiento de pesos y por tanto soporte de esfuerzos, además al tratarse de una malformación de
mano parcial y no una amputación médica, existe poca disponibilidad al respecto, lo cual aumenta
el reto del diseño ya que supone algo novedoso y a empezar desde cero.
Se comienza por la idea de desarrollar una prótesis a modo de gancho, robusta para soportar
las fuerzas solicitadas y con poca movilidad al no ser esta la necesidad para la cual se diseña. Se
observa que una de las limitaciones principales será la sujeción al brazo para que el paciente haga
el esfuerzo necesario sin necesidad de sufrir daño añadido. De esta forma, se plantea un socket que
no solo abarque la palma de la mano sino parte del brazo del paciente.
Para una mayor exactitud en el diseño se realiza un escaneo del brazo del paciente, de forma que
se genera un modelo tridimensional que permite modelar una prótesis completamente acorde a la
morfología de la malformación. A partir de este escaneo se generan una serie de componentes que conformaran la prótesis final, por un lado el socket que recubrirá la extremidad y ejercerá el papel
de interfaz entre el paciente y la prótesis, con vistas a que en un futuro sea posible intercambiar el
terminal en función de la necesidad del paciente. A continuación el terminal en el que se albergará
la electrónica y el mecanismo que aporta movimiento a la entidad, y finalmente el gancho, que se
mueve gracias a la no concentricidad de los ejes que lo atraviesan.
En el aspecto electrónico se utilizan un par de botones cuyas pulsaciones implican apertura y
cierre respectivamente, así como un electroimán cuya finalidad es hacer de cierre de seguridad de
forma que se aporte tranquilidad al paciente durante la realización del ejercicio ya que se evita la
posible apertura inesperada de la prótesis.
Se realiza también un estudio de elementos finitos de aquellas zonas de la prótesis donde recaerán
la mayor parte de los esfuerzos, permitiendo así ayudar al diseñador a observar los puntos débiles
del diseño así como la elección óptima de materiales para cada una de las piezas.
Con la idea de aumentar la comodidad en la sujeción para el paciente, se plantea la posibilidad de
proseguir con un diseño posterior al mostrado en este proyecto, que no solo implique el antebrazo
sino también el resto de extremidad superior, pudiendo incluso tenerse en cuenta un sistema de
sujeción con elásticos en la espalda. | es |
dc.description.abstract | This project arose from the idea that Arturo Moreno Arteaga, student in Health Engineering,
proposed upon realising the difficulty that the author of this report (Álvaro López Tornos)
encountered when doing exercises with bars in the gym, owing to his having suffered Poland
Syndrome since birth. From being basketball teammates for several years and class friends since
childhood, Arturo knows well the limitations that Álvaro has, so, taking advantage of the branch
of engineering in which he specializes, he suggested carrying out the design of a prosthesis that
helps Álvaro perform the activities that he considers most challenging. This led Álvaro to put forward the idea of putting into practice a joint end-of-degree project, each with a different theme, but
with a common nexus: the design of a partial hand prosthesis for gripping cylindrical bars in the gym.
Thus, and observing the myriad of ideas that nowadays are implemented in prostheses, the students
decided to divide their work into, on the one hand, a development in mechanical design (Álvaro),
and, on the other hand, a contribution in electronics that provides functionality to the prosthesis
(Arturo), each theme in conjunction with what each student studied in his respective degree, and
that way both students could benefit from each other’s knowledge.
The field of prosthetics is currently growing exponentially, the development of new technologies,
as well as the variety of amputations and malformations makes the prosthetic diversity in the market
immense. However, only a small number of these prostheses are focused on weight-lifting and
therefore support such efforts. Also, being a partial hand malformation and not a medical amputation,
there isn’t much available for Álvaro, which increases the design challenge, as it involves something
new and starting from scratch.
The design starts with the idea of developing a prosthesis as a hook, robust to withstand the forces
required and with little mobility, as this is not the function for which it is designed. It is observed
that one of the main requirements is to have the prothesis adhere to the arm so that the patient
can make the necessary effort without having to suffer added damage. Hence, a socket has been
developed that not only covers the palm of the hand but also part of the patient’s arm.
For greater accuracy in the design, a scan of the patient’s arm has been performed, generating a
three-dimensional model that allowed modeling a prosthesis completely according to the morphology of the malformation. From this scan, a series of components were generated that form the final
prosthesis, on the one hand the socket that covers the limb and plays the role of interface between the patient and the prosthesis, so that in the future it will be possible to exchange the terminal depending
on the patient’s needs. Then, the upper part which will house the electronics and the mechanism that
provides movement to the entity, and finally the hook, which moves thanks to the non-concentricity
of the axes that pass through it.
In the electronic aspect, a pair of buttons are used, which, when pressed, open and close the
device respectively, as well as an electromagnet, the purpose of which is to act as a safety lock to
provide peace of mind to the patient during the exercise, as it prevents the possible unexpected
opening of the prosthesis.
A finite element study has been done of those areas of the prosthesis where most of the efforts
will fall, thus helping the designer to observe the weak points of the design as well as the optimal
choice of materials for each of the pieces.
With the idea of increasing the comfort of engagement with the patient, the possibility of continuing with a design subsequent to the one shown in this project, which not only involves the forearm
but also the rest of the upper extremity, even considering a support system with elastics in the back,
