Los trastornos hereditarios de la coagulación engloban diversas enfermedades siendo la hemofilia y la enfermedad de von Willebrand, las más frecuentes. Debido a la deficiencia en el organismo de determinados factores de la coagulación, estos pacientes presentan un elevado riesgo de hemorragia, por lo que es necesario planificar adecuadamente la intervención quirúrgica oral, en relación estrecha con el hematólogo para decidir el plan de actuación y las medidas hemostáticas a aplicar. Los objetivos de esta revisión son conocer qué procedimientos quirúrgicos avanzados están indicados o contraindicados en los pacientes con trastornos hereditarios de la coagulación y qué medidas hemostáticas se han de tener en cuenta previo a los procedimientos quirúrgicos avanzados para disminuir el riesgo de complicaciones hemorrágicas. Para la consecución de los objetivos se llevó a cabo una revisión bibliográfica a través de las bases de datos PubMed y Scopus. De los 2838 artículos iniciales, solo 18 artículos fueron incluidos para el análisis. Las conclusiones de la revisión bibliográfica muestran que, aunque existen diferentes tratamientos efectivos para reducir las complicaciones hemorrágicas en estos pacientes, la comunicación directa con el hematólogo es de vital importancia al no existir aún un protocolo definido ante el tratamiento quirúrgico oral avanzado en los pacientes con trastornos hereditarios de la coagulación. Due to the deficiency in the organism of certain coagulation factors, these patients have a high risk of bleeding, so it is necessary to adequately plan the oral surgical intervention, in close relationship with the hematologist to decide the action plan and the hemostatic measures to apply. The objectives of this review are to know what advanced surgical procedures are indicated or contraindicated in patients with inherited bleeding disorders and what 2 hemostatic measures should be taken into account prior to advanced surgical procedures to reduce the risk of bleeding complications. To achieve the objectives, a bibliographic review through PubMed and Scopus databases was carried out. Of the 2838 initial articles, only 18 were included for the analysis. The conclusions of the bibliographic review show that, although there are different effective treatments to reduce bleeding complications in these patients, direct communication with the hematologist is of vital importance since there is still no defined protocol for advanced oral surgical treatment in patients with hereditary bleeding disorders. |
