Las estatinas son medicamentos ampliamente utilizados para reducir el colesterol en pacientes con hiperlipidemia, que actúan como inhibidores selectivos de la 3-hidroxi-3-metilglutaril coenzima A (HMG-CoA) reductasa. Además, son capaces de reducir la incidencia de eventos cerebrovasculares y cardiovasculares, independientemente de su efecto sobre los niveles de colesterol. Existen estudios que han demostrado que estos medicamentos pueden ser beneficiosos para una variedad de enfermedades oculares, entre ellas el glaucoma. El glaucoma es una neuropatía óptica asociada con la pérdida del campo visual, y se considera la principal causa de ceguera irreversible en todo el mundo. Debido a que el daño del nervio óptico es progresivo e irreversible, es importante un diagnóstico precoz y un tratamiento rápido. La presión intraocular (PIO) elevada es el principal y único factor de riesgo modificable del glaucoma de ángulo abierto (GAA), la forma más común de glaucoma. Actualmente, el tratamiento se dirige exclusivamente hacia la disminución de la PIO, sin embargo, la progresión de la enfermedad a menudo continúa a pesar de una reducción aparentemente suficiente de esta. Por esta razón, es necesaria la búsqueda de nuevos agentes terapéuticos que ofrezcan protección a los pacientes frente a la pérdida visual glaucomatosa. Así pues, en esta revisión bibliográfica se examina la evidencia clínica y epidemiológica actual que investiga la asociación entre el uso de estatinas y la incidencia o progresión del glaucoma, y sus efectos sobre la PIO. Se han propuesto varios mecanismos protectores de estos medicamentos que se relacionan con un riesgo reducido de desarrollar glaucoma. Además, se estudia el efecto de otros hipolipemiantes y otros tipos de medicación sistémica en la enfermedad. |
