España; European Regional Development FundToday great hope is set on regenerative medicine in all medical fields. Leland Kaiser introduced the term “Regenerative Medicine” in 1992. He forecasted that a “new branch of medicine will develop that attempts to change the course of chronic diseases and in many instances will regenerate tired and failing organ systems” (Kaiser, 1992). Since then, scientists all over the world try to develop cell-based approaches to regenerate damaged tissues, or even substitute whole organs (Ehnert et al., 2009). Degenerative disease of articular cartilage (AC), generically known as osteoarthritis (OA), is an irreversible evolution process towards terminal articular failure. Due to its high prevalence on population and its socioeconomic impact, this condition is of great concern, and this way more resources and effort are dedicated to the research on its development. Cartilage tissue engineering seeks to combine cells, biomaterial scaffolds, and bioactive signals to create functional tissue replacements to treat cartilage injuries or osteoarthritis (Song et al., 2004). Cartilage degenerative disease, OA, is the end stage of several conditions such as trauma, inflammatory diseases, overweight etc. The fatalistic theory that states that it is impossible to recover the cartilage once it has been damaged leads to the assumption that the progression to any form of OA is unavoidable (Fig. 1). The annual incidence of young adults suffering any cartilage injury in UK is 10000 and this figure is continuously increasing (NICE, 2008).

Leptin is now considered an important signaling molecule of the reproductive system, since it regulates the production of gonadotropins, the blastocyst formation and implantation, the normal placentation, as well as the fetoplacental communication. Placental leptin is an important cytokine which regulates placental functions in an autocrine or paracrine manner. Leptin seems to play a crucial role during the first stages of pregnancy as it modulates critical processes like proliferation, protein synthesis, invasion, and apoptosis in placental cells. Furthermore, deregulation of leptin levels has been correlated with the pathogenesis of various disorders associated with reproduction and gestation, including gestational diabetes mellitus (GDM). Due to the relevant incidence of the GDM and the importance of leptin, we decided to review the latest information available about leptin action in normal and GDM pregnancies to support the idea of leptin as an important factor and/or predictor of diverse disorders associated with reproduction and pregnancy.

However, some special situations may limit the use of this method due to the difficulty to locate the gland and perform the lesion excision. We analyze 2 patients of a left adrenal tumor, explaining how they have overcome the difficulties in both situations. The first case was a patient with a history of intra-abdominal surgery and the other patient suffered from severe obesity. We performed with the use of the gamma probe, and the 2 cases, was of great help to access and glandular localization. The help of gamma probe test was achieved in the surgical bed, that removal was complete. The use of the portable gamma probe facilitated the access to the left adrenal gland as well as conducting the glandular excision without delay, despite the difficulties due to the intra abdominal surgery caused by the previous surgery, and in the case of severe obesity.
Artículo Autoinjertos esplénicos. Su influencia en la prevención de sepsis postesplenectomía por Escherichia coli(Paz Montalvo, 1989) Capitan-Morales, Luis-Cristobal; García Iglesias, Mercedes Concepción; García Diáz, F.; Jiménez-García, Antonio; Cantillana Martínez, J.; Loscertales Abril, Jesús; CirugíaSe presenta un estudio experimental de la eficacia de autoinjertos esplénicos frente a una sepsis inducida por Escherichia coli. Se han intervenido 75 ratas Wistar distribuidas en tres grupos: I) intervención simulada; II) esplenectomía total, y III) autoinjerto esplénico. A los noventa días de la intervención se efectuó una inoculación con el microorganismo realizando hemocultivos a los diez y ciento veinte minutos. Los resu11adOs muestran que el aclaramiento bacteriano no aumentó en los animales autoinjertados, siendo los trasplantes ineficaces para combatir una sepsis por Escherichia coli.

Se especifican sus indicaciones contraindicaciones y resultados que podemos esperar. Destacamos la consistencia de los resultados obtenidos, al presentarse un seguimiento de 2’5 años.
