Similitudes y diferencias entre la fibromialgia y los trastornos temporomandibulares

Abstract

OBJETIVO: revisar la literatura existente para poder determinar las similitudes y diferencias que se encuentran entre la etiología, fisiopatología, clínica y tratamiento de la fibromialgia y los trastornos temporomandibulares.  METODOLOGÍA DE LA BÚSQUEDA Y MATERIAL: Se realizó una búsqueda bibliográfica en la base de datos PubMed y Scopus de acuerdo a las palabras claves seleccionadas, en función de los términos MesH y de acuerdo a unos criterios de selección. Dichos artículos fueron analizados para establecer los resultados de este estudio.  ESTRATEGIA DE BÚSQUEDA: (Similarities OR differences) AND Fibromyalgia AND (“Temporomandibular joint disorder” OR “Temporomandibular joint dysfuction”)  RESULTADOS: En PubMed se encontraron 20 artículos de acuerdo a la estrategia de búsqueda, y aplicando los criterios de inclusión y exclusión, fueron seleccionados 7 artículos que cumplían los objetivos del estudio. o Esta misma búsqueda en SCOPUS tuvo como resultado 36 artículos de los cuales 15 eran repetidos. o Se añadieron 15 artículos del apartado de artículos relacionados que ofrece PubMed.  CONCLUSIÓN: Existen similitudes y diferencias en la etiología, clinica y tratamiento de ambas patologías. Encontramos que la fibromialgia predispone la aparición de los trastornos temporomandibulares. Ambas patologías pueden ser afectadas por factores medioambientales, trastornos del sueño, respuesta neuromuscular anormal y sensibilización central. Se diferencian en la prevalencia de los puntos gatillos y la ubicación de las zonas de dolor referido. Y como tratamiento se aconseja llevar un estilo de vida más saludable tanto física como socialmente, además de una terapia láser y una férula oclusal en los trastornos temporomandibulares.

OBJETIVE: To review the available literatura in order to determine the similarities and differences among the etiology, fisiopathology, signs and symptoms and treatment of fibromyalgia and temporomandibular joint disorder.  SEARCH METHODOLOGY AND MATERIAL: a bibliographic reaserch was done in PubMed and SCOPUS databases according to the selected keywords, some MesH terms and selection criteria. These articles were analized to stablish the results of the present study.  SEARCH STRATEGY: (Similarities OR differences) AND Fibromyalgia AND (“Temporomandibular joint disorder” OR “Temporomandibular joint dysfuction”)  RESULTS: we found 20 articles in PubMed according to the search strategy, and after applying the inclusion and exclusion criteria, we selected 7 articles that fulfilled the objetives of the study. The same search in SCOPUS resulted in 36 articles of which 15 were repeated. o 15 articles were added from the section of related articles that appears in PubMed.  CONCLUSION: There are similarities and differences in the etiology, clinic and treatment of both pathologies. We found that fibromyalgia predisposes the appearance of temporomandibular disorders. Both pathologies may be affected by environmental factors, sleep disorders, abnormal neuromuscular response and central sensitization. They differ from the prevalence of trigger points and the location of referred pain zones. And as a treatment it is advised to lead a healthier lifestyle both physically and socially, in addition to a laser therapy and an occlusal splint in temporomandibular disorders 
