dc.description.abstract | Introducción: Fumar afecta negativamente a las comunidades microbianas subgingivales, favoreciendo un microbioma periimplantario rico en patógenos aun en estados de salud clínica. Y se conoce que la presencia de bacterias es un factor importante para establecer la pérdida ósea periimplataria y la estabilidad de un implante a largo plazo. Objetivos: conocer si el tabaco es un factor de riesgo de periimplantitis, si es una contraindicación absoluta para la colocación de implantes, conocer los cambios en el microbioma periimplantario de los pacientes fumadores, el impacto de los componentes tabáquicos en el tejido periimplantar, conocer si la localización del implante aumenta el riesgo de periimplantitis y describir los diferentes tratamientos de la periimplantitis. Material y métodos: se realizó una búsqueda bibliográfica exhaustiva en las bases de datos, Pubmed-MEDLINE y Google Scholar. Esta revisión se basa en 35 estudios. Resultados: el microbioma periimplantar de pacientes fumadores se ve alterado con respecto a pacientes no fumadores. Los componentes tabáquicos tienen efectos a nivel periimplantar y molecular. Los parámetros clínicos periimplantares son más desfavorables en maxilar que en mandíbula. Los parámetros clínicos periimplantarios en pacientes fumadores de cigarrillos, pipa de agua o cigarrillos electrónicos son más desfavorables que en pacientes no fumadores y no estadísticamente significativos entre ellos. Y el tratamiento más eficaz para tratar la periimplantitis es el desbridamiento mecánico y terapia fotodinámica. Conclusiones: El tabaco si es un factor de riesgo de la periimplantitis pero no se considera una contraindicación para la colocación de implantes. El tabaquismo altera el microbioma a diferentes niveles, modificando potencialmente el medio periimplantario, y la funcionalidad y patogenicidad bacteriana. Los componentes tabáquicos producen cambios a nivel molecular y periimplantar. El maxilar es más permeable a los agentes perjudiciales del humo y tiene mayor riesgo de sufrir periimplantitis. Y los tratamientos más utilizados contra la periimplantitis son el desbridamiento mecánico solo o con terapia fotodinámica complementaria. | es |
dc.description.abstract | Introduction: Smoking negatively affects subgingival microbial communities, favoring a peri-implant microbiome rich in pathogens even in clinical health states. And the presence of bacteria is known to be an important factor in establishing peri-implant bone loss and long-term implant stability. Objectives: to know if tobacco is a risk factor for peri-implantitis, if it is an absolute contraindication for implant placement, to know the changes in the peri-implant microbiome of smoking patients, the impact of tobacco components on peri-implant tissue, to know if implant location increases the risk of peri-implantitis and describe the different treatments for periimplantitis. Material and methods: an exhaustive bibliographic search was carried out in the databases, Pubmed-MEDLINE and Google Scholar. This review is based on 35 studies. Results: the peri-implant microbiome of smoking patients is altered compared to non-smoking patients. Tobacco components have effects at the peri-implant and molecular level. The periimplant clinical parameters are more unfavorable in the maxilla than in the mandible. The peri-implant clinical parameters in patients who smoke cigarettes, water pipes or electronic cigarettes are more unfavorable than in non-smokers and not statistically significant among them. And the most effective treatment for treating peri-implantitis is mechanical debridement and photodynamic therapy. Conclusions: Tobacco is a risk factor for periimplantitis but it is not considered a contraindication for implant placement. Smoking alters the microbiome at different levels, potentially modifying the peri-implant environment, and bacterial functionality and pathogenicity. Tobacco components produce changes at the molecular and peri-implant level. The maxilla is more permeable to the damaging agents of smoke and has a higher risk of peri-implantitis. And the most used treatments against peri-implantitis are mechanical debridement alone or with complementary photodynamic therapy. | es |
