Review of the surgical treatment of corono radicular syndesmus

Abstract

Introduction. The corono radicular syndesmus is a dental morphological anomaly that particularly affects the upper lateral incisors with a groove that goes from coronal to root with different extensions. This leads to accumulation of plaque, periodontal involvement with bone destruction and retro contamination of the root canal in the most complex cases. Being necessary a surgical approach to the tooth and root canal treatment in most cases. Material and methods. A search was carried out on the surgical treatment of the crown root syndesmus, better known as the palatoradicular groove, in which 14 articles were finally selected with clinical cases that exposed their experiences with the treatments carried out. Conclusions. Complicated root syndesmus lesions require a clinical approach in which root canal treatment is combined if necessary, although treatment must be focused primarily on eliminating the sulcus. To access the root portion of the tooth we have two ways, through a mucoperiosteal flap that has given very good results or by intentional reimplantation of the tooth, which is an alternative with a worse prognosis. Both techniques are aimed at accessing the root portion of the tooth and being able to perform odontoplasty, apicoectomy and/or filling of the sulcus with different biocompatible materials to stop being the cause of inflammation.

Introducción. El sindesmo corono radicular en una anomalía morfológica dentaria que particularmente afecta a los incisivos laterales superiores con un surco que va de coronal a radicular con diferentes extensiones. Éste da lugar a acúmulo de placa, afectación periodontal con destrucción ósea y contaminación a retro del canal radicular en los casos más complejos. Siendo necesario un abordaje quirúrgico del diente y el tratamiento de conductos en la mayoría de los casos. Material y métodos. Se hizo una búsqueda sobre el tratamiento quirúrgico del sindesmo corono radicular, más conocido como surco palato radicular, en la que finalmente se seleccionó 14 artículos con casos clínicos que exponían sus experiencias con los tratamientos realizados.
