Abstract | La farmacocinética, entendida como el estudio y caracterización de la evolución temporal de los fármacos y de sus metabolitos en el organismo, ha evolucionado enormemente. Sin embargo, debido a los continuos cambios ... La farmacocinética, entendida como el estudio y caracterización de la evolución temporal de los fármacos y de sus metabolitos en el organismo, ha evolucionado enormemente. Sin embargo, debido a los continuos cambios fisiológicos/bioquímicos del organismo pediátrico, que influyen considerablemente en los procesos farmacocinéticos, estos estudios no son habituales en la población infantil. Por ello, lo más frecuente es recurrir a la extrapolación de los datos obtenidos en adultos, y admitir que los efectos adversos son similares en ambos grupos de población. Para evidenciar estos hechos se realizó este trabajo donde se puntualizan las diferencias más relevantes entre la farmacocinética de ambas poblaciones; así como las repercusiones de esta farmacocinética infantil en la administración de algunos fármacos. Se enumeran primeramente una serie de características propias para los cinco subgrupos en los que se divide a la infancia, con el fin de entender mejor la farmacocinética en ambas poblaciones. En la población pediátrica la velocidad y el grado de absorción son variables. El efecto del primer paso se reduce alcanzando los valores de adultos en bebés. Una adecuada unión a proteínas plasmáticas ocurre a los 10-12 meses de vida. El volumen de distribución difiere entre ambas poblaciones, debido a las diferencias en agua y grasa corporal. La metabolización diverge debido a la maduración diferencial de los procesos hepáticos. La excreción renal es menor en los recién nacidos a causa de la inmadurez en la función renal. Así: a) como consecuencia de estar limitada la actividad del enzima CYP2D6 durante el primer año, la eficacia analgésica del tramadol se ve afectada; b) debido a los cambios en los parámetros farmacocinéticos en el Propofol, los niños muestran menor sensibilidad a la anestesia; c) la nefrotoxicidad de los aminoglucósidos conlleva su administración limitada; d) la farmacogenética provoca absorción y eliminación muy variable de la rifampicina. |
