La doxorrubicina es una antraciclina que se utiliza para el tratamiento del cáncer. Plantea desventajas potenciales tales como la toxicidad debido a la baja permanencia plasmática y distribución indiscriminada en el organismo, que hace que las células sanas resulten también afectadas. También provoca incapacidad de alcanzar concentraciones lo suficientemente elevadas en el sitio de acción y fenómenos de resistencia. En base a esto, surge la doxorrubicina liposomal no pegilada que a pesar de tener ventajas con respecto a doxorrubicina convencional, presenta el inconveniente de la captación de liposomas por el sistema fagocítico mononuclear que impide su distribución en los tumores sólidos por su eliminación rápida de la circulación. Entonces, se desarrolla la doxorrubicina liposomal pegilada que resuelve el problema de la rápida eliminación aumentando su vida media en la sangre. Ambas formulaciones aumentan el aporte en la localización de la enfermedad en comparación con la dosis equivalente a doxorrubicina convencional. Pero el reto sigue siendo lograr la acumulación de doxorrubicina liposomal en el tejido tumoral, por ello la doxorrubicina se conjuga con inmunoliposoma. Para alcanzar estos objetivos de forma específica se diseñan nanopartículas recubiertas con polímeros, gracias a los cuales permanecen en el torrente circulatorio sin ser eliminadas y aumentan la acumulación de doxorrubicina en el tejido tumoral. Este tipo de nanopartículas presentan mayores ventajas que las formas liposomales ya que superan y aumentan la selectividad de doxorrubicina hacia las células tumorales, mientras que reducen la toxicidad en los tejidos sanos. Producen gran apoptosis del tejido tumoral y también mejoran la resistencia del fármaco convencional. Por otro lado, se han desarrollado también micelas conjugadas con doxorrubicina que al igual que las nanopartículas anteriores destaca entre ellas su gran captación intracelular, permeabilidad, resistencia, menor citotoxicidad y mayor capacidad de inhibición del ciclo celular que la doxorrubicina convencional |
