Abstract | Objetivo: analizar el éxito de la eliminación selectiva de la caries profunda en dientes
permanentes y determinar si es beneficioso dejar dentina afectada en la cavidad. Se busca

Objetivo: analizar el éxito de la eliminación selectiva de la caries profunda en dientes permanentes y determinar si es beneficioso dejar dentina afectada en la cavidad. Se busca comprender el comportamiento y la evolución de la caries blanda de dentina con la técnica de eliminación selectiva, y comparar la eficacia de la técnica tradicional de eliminación hasta dentina dura con la técnica de eliminación selectiva de la caries hasta dentina blanda. Material y método: se realiza una búsqueda en Web of Science empleando las palabras claves Excavation OR Caries removal OR Caries excavation OR Caries-excavation OR Guide of carious dentin removal OR Cavity preparation OR Stepwise excavation OR Caries OR Arrested caries OR Boundary caries OR Caries detection. Deep caries OR Cavity caries OR carious lesions OR carious limit’’ teniendo en cuenta los criterios de inclusión. Resultados: se obtuvieron 1236 resultados de los cuales finalmente se seleccionaron 18. Conclusiones: La exploración visual es la técnica más común, pero tiene limitaciones para detectar caries en áreas profundas. La técnica de eliminación en dos etapas conserva la dentina sana al eliminar solo la capa superficial de la caries y dejar una capa subyacente que se trata posteriormente. La eliminación con láser puede ser una técnica efectiva y conservadora para la eliminación de caries, con menor daño a la dentina y mejor adhesión de la resina compuesta. El uso de cemento bioactivo y solución de tinte bioactivo muestra beneficios en la reducción de la carga bacteriana y la detección temprana de la caries dental.La transiluminación en el infrarrojo cercano es una técnica prometedora para la detección de lesiones cariosas. La técnica químico-mecánica puede ser una alternativa efectiva a la remoción convencional total de caries dentales profundas, con resultados similares en términos de éxito del tratamiento y reducción de síntomas de dolor y sensibilidad en los pacientes.Es importante destacar que se necesita más investigación para confirmar y evaluar la eficacia a largo plazo de estas técnicas en diferentes entornos clínicos y usos. The aim is to understand the behavior and evolution of soft dentin caries with the selective removal technique, and to compare the efficacy of the traditional removal technique up to hard dentin with the selective removal technique of caries down to soft dentin. Deep caries OR Cavity caries OR carious lesions OR carious limit'' taking into account the inclusion criteria. Results: 1236 results were obtained, of which 18 were finally selected. Conclusions: Visual exploration is the most common technique, but it has limitations to detect caries in deep areas. The two-stage removal technique preserves healthy dentin by removing only the superficial layer of caries, leaving an underlying layer that is subsequently treated. Laser removal can be an effective and conservative technique for caries removal, with less damage to dentin and better composite resin adhesion. The use of bioactive cement and bioactive dye solution shows benefits in reducing bacterial load and early detection of dental caries.Near-infrared transillumination is a promising technique for the detection of carious lesions. The chemical-mechanical technique can be an effective alternative to the conventional total removal of deep dental caries, with similar results in terms of treatment success and reduction of pain and sensitivity symptoms in patients.Importantly, more research is needed to confirm and evaluate the long-term efficacy of these techniques in different clinical settings and uses. |
