los procedimientos metodológicos usados en las ... INTRODUCCIÓN: La precisión de los localizadores de ápice en la determinación de la longitud de trabajo (LT) varia de manera importante (50-100%). Esto puede deberse a los procedimientos metodológicos usados en las diferentes publicaciones de la literatura. La constricción apical (CA) como punto final de la preparación, así como el estudio de cómo afectan los irrigantes a la precisión de los localizadores, son factores importantes a tener en cuenta. La hipótesis nula fue que los artículos publicados a partir del 2014 no contenían sesgos en su metodología. OBJETIVOS: Establecer si desde el 2014 y hasta mayo de 2023, las nuevas investigaciones cumplen con los parámetros de la odontología basada en la evidencia y los estrictos criterios de inclusión con respecto a la precisión de los localizadores electrónicos de ápice (LEAs) en la determinación de LT. MATERIAL Y MÉTODOS: Se llevó a cabo una búsqueda sistemática de la literatura para identificar estudios que evaluaran histológicamente la precisión de los LEAs en dientes humanos. Los estudios identificados estaban sujetos a criterios de inclusión estrictos seguidos por la extracción de datos y el análisis. RESULTADOS: De 138 artículos, solo 5 cumplían con los criterios de elegibilidad establecidos, además 2 presentaban bajo riesgo de sesgo y 3 alto riesgo de sesgo. La distancia media de la punta de la lima a CA (L-CA) fue de 0,32 mm y se estudiaron un total de 5 localizadores de ápice. La presencia de hipoclorito de sodio, así como de sangre, podía influir en las medidas obtenidas por los localizadores. CONCLUSIONES: Aún, el volumen de estudios sin sesgo o con bajo riesgo de sesgo es poco. De las investigaciones estudiadas, se concluye que los localizadores de ápice son un medio fiable para establecer la longitud de trabajo, siendo la marca 0,5 más precisa. El hipoclorito de sodio y la sangre en el conducto radicular, genera medidas mayores en los localizadores. Son necesarios más estudios que clarifiquen la influencia de determinadas variables en la precisión de estos instrumentos. Apical constriction as the endpoint of preparation, as well as the study of how irrigants affect the accuracy of locators, are important factors to consider. The null hypothesis was that articles published from 2014 onwards did not contain biases in their methodology. OBJECTIVES: To determine if, from 2014 to May 2023, new research meets the parameters of evidence-based dentistry and strict inclusion criteria regarding the accuracy of Electronic Apex Locators (EALs) in determining the working length (WL). MATERIAL AND METHOD: A systematic search of the literature was performed to identify studies that histologically evaluated the precision of EALs in human teeth. The identified studies were subject to strict inclusion criteria followed by data extraction and analysis. RESULTS: Out of 138 articles, only 5 met the established eligibility criteria, 2 of then have a low risk of bias and 3 have a high risk of bias. The mean distance from the file tip to the apical constriction was 0.32 mm, and a total of 5 apex locators were studied. The presence of sodium hypochlorite and blood could influence the measurements obtained by the locators. CONCLUSIONS: Currently, the volume of studies without bias or with low risk of bias is limited. From the analyzed research, it can be concluded that apex locators are a reliable means to establish the working length, with the 0.5 mark being more accurate. Sodium hypochlorite and blood in the root canal generate larger measurements on the locators. Further studies are needed to clarify the influence of certain variables on the accuracy of these instruments. |
