El síndrome del intestino irritable (SII) es un trastorno funcional multifactorial del tracto digestivo que afecta a una creciente proporción de la población adulta. La consecuente morbilidad derivada de las secuelas del SII genera importantes repercusiones económicas y sociales. Factores alimenticios, hábitos de vida incorrectos, la somatización de algunos sucesos traumáticos, el estado emocional del individuo... son elementos coexistentes que explican una disfunción del aparato gastrointestinal ante la ausencia de una alteración orgánica o fisiológica visible. Todos los tratamientos desarrollados en la actualidad se encaminan a resolver estos factores predisponentes al síndrome. La hipersensibilidad visceral, entendida como la percepción anómala del dolor visceral, desempeña un papel clave en la fisiopatología del SII. Como consecuencia, el intestino se encuentra en una situación de hiperexcitabilidad y de «defensa constante», con lo que el sistema visceral y, consecuentemente, las cadenas musculares se ven afectadas. El objetivo del presente estudio es proponer, de forma teórica, una nueva vía de tratamiento basada en resolver la alteración visceral propia del SII empleando como base la reeducación de las cadenas viscerales propuesta por Léopold Busquet. Material y método: se ha llevado a cabo una revisión bibliográfica y en diferentes bases de datos (pubmed, elsevier, cochrane, etc.) empleando irritable bowel syndrome (IBS), physiopathology y visceral como palabras clave. Resultados: la sensación de inflamación intestinal aparece en torno al 95 % de los individuos afectos de trastornos gastrointestinales funcionales. Sin embargo, esta «sensación» no siempre se corresponde con un verdadero incremento del perímetro abdominal, sino que en cerca del 50 % de los casos, el diámetro abdominal se encuentra disminuido debido a la necesidad de la «defensa» de las vísceras ante un estado de hiperexcitabilidad continuada. Acorde con lo expuesto por Busquet, el aumento o disminución del diámetro del abdomen da origen a lo que se conoce como despliegue o repliegue visceral, respectivamente. En función de la adaptación visceral acontece un conjunto de compensaciones posturales que delimitan un nuevo patrón de posición en el sujeto. Conclusiones: el análisis postural individual de cada sujeto con SII permitiría determinar con precisión la adaptación visceral abdominal (ya sea repliegue o despliegue) que se ha producido en su caso concreto y ante ello establecer el programa fisioterapéutico necesario. |
