Con el avance en el conocimiento de las herramientas de edición génica, la posibilidad de producir modificaciones específicas en el genoma está experimentando progresos significativos en las últimas décadas, permitiendo generar posibles tratamientos curativos para enfermedades genéticas que se consideraban irremediables. El sistema CRISPR-Cas9 identificado en bacterias y constituido por un ARN guía (ARNg) y la nucleasa Cas9, ha revolucionado el campo de la edición génica al ser capaz de modificar el genoma de manera precisa, eficiente y sencilla. En el contexto de la anemia falciforme, una enfermedad monogénica causada por una mutación puntual en el gen HBB, el sistema CRISPR-Cas9 muestra un gran potencial como posible tratamiento curativo. Esta tecnología permite abordar directamente la causa de la enfermedad, generando un cambio que devuelve a los glóbulos rojos su función normal. En esta revisión bibliográfica, se analizarán las distintas opciones terapéuticas actuales empleadas en el tratamiento de la anemia falciforme y sus posibles efectos, además de una visión general del estado actual de los ensayos clínicos que utilizan la edición genética CRISPR-Cas9 y las posibilidades que nos ofrecen. With advancements in the understanding of gene editing tools, the ability to produce specific modifications in the genome has experienced significant progress in recent decades, allowing for the generation of potential curative treatments for genetic diseases that were once considered incurable. The CRISPR-Cas9 system, identified in bacteria and consisting of a guide RNA (gRNA) and the Cas9 nuclease, has revolutionized the field of gene editing strategies by being capable of precisely, efficiently, and simply modifying the genome. In the context of sickle cell disease, a monogenic disease caused by a point mutation in the HBB gene, the CRISPR-Cas9 system shows great potential as a possible curative treatment. This technology enables direct targeting of the underlying cause of the disease, inducing a change that restores normal function to red blood cells. In this review, we will analyse the different current therapeutic options employed in the treatment of sickle cell disease and their potential effects, and 4 provide an overview of the current state of clinical trials using CRISPR-Cas9 gene editing and the possibilities it offers. |
