Síndrome de malabsorción en el niño: niveles plasmáticos de folatos y vitamina B12. Correlaciones anatomoclínicas

Abstract | Hemos estudiado 100 pacientes con disturbios gastrointestinales, que hacían sospechar síndrome de malabsorción, realizándose las siguientes técnicas: a) 115 biopsias intestinales con cápsula de Crosby-Kugler, valoradas ... Hemos estudiado 100 pacientes con disturbios gastrointestinales, que hacían sospechar síndrome de malabsorción, realizándose las siguientes técnicas: a) 115 biopsias intestinales con cápsula de Crosby-Kugler, valoradas según los criterios de Whitehead. B) 115 niveles séricos de folatos, realizados según técnica de Ria. C) 114 niveles séricos de vitamina b12 según técnica de Ria. D) 72 curvas de xilosa, con medida de xilosemia en sangre capilar a los 60 minutos, según la técnica de Roe y Rice. Los criterios de exclusión son anticonvulsivos, citostáticos, complejos vitamínicos, antibióticos y quimioterápicos, si se han recibido en el mes previo a los análisis. El estudio estadístico se realizó con una base de datos (base III), y procesado con el programa SPSS en un ordenador de Ibm. Se ha realizado descriptiva general y por grupos, test de Kolmogorov-Smirnov, análisis de varianza, t-student, y análisis de regresión múltiple. Resultados: en la t-student no existe diferencia significativa entre vitamina b12 xilosemia a los 60 minutos y folatos séricos respecto a los grados de alteración de la biopsia. En el análisis de regresión múltiple existe diferencia significativa, entre xilosemia a los 60 minutos, y folatos séricos, en biopsia normal; así como entre xilosemia a los 60 minutos y vitamina b12, y vitamina b12 y folatos séricos en la biopsia anormal. Como resumen de las conclusiones podemos decir que ninguno de los tres parámetros ha sido útil para conocer el estado de la mucosa; deben valorarse poblaciones sanas para ver si el punto de inflexión de xilosemia es 20 o 30 mg/dl; ninguna de las tres pruebas puede sustituir a la biopsia, y si existen contradicciones debe prevalecer la biopsia; se deben realizar estudios mas amplios con estos parámetros, antes de emitir un juicio definitivo sobre estos test. |
