infecciones endodóncicas.

Objetivos: Determinar el conocimiento y hábitos de los odontólogos generales españoles sobre las indicaciones de antibióticos sistémicos en el tratamiento de las infecciones endodóncicas. Material y métodos: En este estudio transversal descriptivo observacional se solicitó a 200 odontólogos generales españoles que respondieran a una encuesta, validada previamente en otras investigaciones publicadas, sobre indicaciones de antibióticos sistémicos en el tratamiento de infecciones endodóncicas. Los datos fueron analizados mediante el uso de estadísticas descriptivas y la prueba de Chi-cuadrado de Pearson. Resultados: 190 odontólogos generales (95%) respondieron a la encuesta satisfactoriamente y fueron incluidos en este estudio. La duración promedio de la terapia con antibióticos fue de 6,51 ± 1,0 días. En pacientes sin alergias médicas, la mayoría (97 %) seleccionaron la amoxicilina como el antibiótico de primera elección, sola (51 %) o asociada con ácido clavulánico (46 %); El fármaco de primera elección para pacientes con alergia a las penicilinas fue clindamicina 300 mg (70 %). Para casos de pulpitis irreversible sintomática, el 44 % de los encuestados recetaron antibióticos. Para casos de necrosis pulpar con periodontitis apical sintomática sin absceso hasta el 35% de los odontólogos generales prescriben antibióticos. Casi el 39 % de los encuestados prescriben antibióticos en casos de necrosis pulpar con periodontitis apical asintomática con fístula. En el escenario de la prescripción antibiótica profiláctica, hasta un 27% de los odontólogos generales prescriben siguiendo pautas no actuales (1 gr 1 hora antes ó 1 gr 1 hora antes y 1 gr 1 hora después) en casos no indicados (16 % en pacientes que toman bifosfonatos orales). Conclusiones: Es necesario mejorar el conocimiento y hábitos de prescripción de antibióticos de los odontólogos generales españoles en endodoncia debido a que muchos prescriben antibióticos de forma errónea. Materials and Methods: In this descriptive observational cross-sectional study, 200 Spanish general dentists were asked to respond to a survey, previously validated in other published research, on indications for systemic antibiotics in the treatment of endodontic infections. Data were analyzed using descriptive statistics and Pearson's Chi-square test. Results: 190 general dentists (95%) responded to the survey satisfactorily and were included in this study. In patients without medical allergies, most (97 %) selected amoxicillin as the antibiotic of first choice, either alone (51 %) or associated with clavulanic acid (46 %); the drug of first choice for patients with penicillin allergy was clindamycin 300 mg (70 %). For cases of symptomatic irreversible pulpitis, 44 % of the respondents prescribed antibiotics. For cases of pulp necrosis with symptomatic periodontitis apicalis without abscess up to 35 % of general dentists prescribe antibiotics. Almost 39 % of the respondents prescribe antibiotics in cases of pulp necrosis with asymptomatic periodontitis apicalis with fistula. In the scenario of prophylactic antibiotic prescription, up to 27 % of the general dentists prescribe according to non-current guidelines (1 gr 1 hour before or 1 gr 1 hour before and 1 gr 1 hour after) in non-indicated cases (16 % in patients taking oral bisphosphonates). Conclusions: There is a need to improve the knowledge and antibiotic prescribing habits of Spanish general dentists in endodontics because many prescribe antibiotics incorrectly. The excessive and inappropriate use of antibiotics is a major contributor to the growing global public health problem of antibiotic resistance. |
