Abstract | Introducción: el cáncer es un conjunto de enfermedades, que se caracteriza por la desregulación de los procesos de proliferación celular debido a mutaciones en el material genético, que dan lugar a células cancerosas que ... Introducción: el cáncer es un conjunto de enfermedades, que se caracteriza por la desregulación de los procesos de proliferación celular debido a mutaciones en el material genético, que dan lugar a células cancerosas que se dividen sin control, formando tumores, e invadiendo tejidos y órganos. El cáncer se encuentra solo por detrás de las enfermedades cardiovasculares como principal causa de muerte en el mundo. Existen distintos tratamientos como la cirugía, quimioterapia, radioterapia, e inmunoterapia. Dentro de la inmunoterapia se encuentra la transferencia celular adoptiva, la cual incluye las células CAR-T. Objetivo: las CAR-T han tenido un gran avance en investigación en los últimos años, por ello se justifica la realización de dicha revisión bibliográfica, teniendo como objetivo conocer en qué consiste, sus indicaciones y limitaciones. Metodología: para la redacción del trabajo se recurrió a artículos, páginas webs oficiales de instituciones oncológicas, agencias de medicamentos… Resultados: la terapia CAR-T consiste en la extracción de linfocitos T a los pacientes mediante aféresis, para posteriormente manipularlos genéticamente ex vivo generalmente con vectores virales y así dotarlos con un CAR, con un receptor antigénico quimérico. El dominio extracelular del receptor se trata de un scFv, capaz de reconocer a un antígeno tumoral específico, unirse a él, desencadenar una respuesta celular, y acabar con la célula cancerosa. Actualmente esta terapia ha tenido mayor éxito en neoplasias hematológicas y no tanto en tumores sólidos. De hecho, no está comercializada aún ningún CAR-T para tumores sólidos, y sí para neoplasias hematológicas. Los dos CAR-T industriales aprobados son para pacientes de hasta 25 años con LLA-B, y para algunos tipos de LNH en adultos. Esta terapia tiene reacciones adversas que pueden llegar a ser graves como el SLS y el ICANS. Conclusión: es necesario seguir investigando para mejorar la eficacia y seguridad de esta costosa terapia. |
