Early Myocardial Strain Reduction and miR-122-5p Elevation Associated with Interstitial Fibrosis in Anthracycline-Induced Cardiotoxicity

Echocardiographic myocardial strain is crucial for early detection of anthracycline-induced cardiotoxicity, particularly in patients at moderate or high risk. Background/ Objectives: This study investigates changes in global longitudinal strain (GLS) in breast cancer patients with low baseline risk for cardiotoxicity during cancer therapy. We also examined the relationship between echocardiographic strain, structural myocardial changes, and microRNA (miRNA) dysregulation associated with cancer treatment using an animal model. Methods: Echocardiography and blood tests were examined in 33 breast cáncer patients with low baseline risk for cardiotoxicity during anthracycline treatment, with a follow-up at 12 months. Additionally, 16 Wistar rats received epirubicin (20 mg/kg over 4 weeks) to examine cardiac strain and structural changes. Moreover, circulating miRNA levels were assessed in patients’ serum using microarray at the end of the treatment and further analyzed in peripheral blood from the animal model. Results: Pathological GLS values were observed in 27.27% of patients after four cycles, with 15.15% showing reduced left ventricular ejection fraction (LVEF) after 12 months. In the animal model, epirubicin-induced circumferential strain (CS) decrease correlates with myocardial fibrosis assessed histologically and by a significant increase in COL1 and TGFB2 expression. Furthermore, we found a significant decrease in aquaporin1 expression associated with the presence of vacuoles in treated rats. Furthermore, dysregulation in the expression of miRNAs was observed in patients with cardiotoxicity. Among them, hsa-miR-122-5p is increased in both patient and rat serum post-treatment. Conclusions: A notable percentage of low-risk patients exhibited cardiac strain reduction due to cardiotoxicity. Epirubicin treatment caused structural heart changes in rats, highlighting miR-122-5p as a potential fibrosis marker that correlated with echocardiographic parameters.

Diseño de una instalación fotovoltaica de autoconsumo con almacenamiento en el término municipal de Beas

Este Trabajo de Fin de Grado, titulado "Diseño de una instalación fotovoltaica con almacenamiento en el término municipal de Beas", presenta de forma integral el desarrollo teórico y práctico de un proyecto de autoconsumo eléctrico basado en energía solar. En él se aborda desde la introducción de los fundamentos de las energías renovables y el impacto de la transición energética en España, hasta la selección y dimensionamiento de todos los componentes de la instalación fotovoltaica a realizar así como su impacto económico. El contenido se estructura en varias secciones: en primer lugar, se analiza el panorama energético nacional, destacando el creciente aporte de las energías renovables, especialmente la solar fotovoltaica, y se explican aspectos básicos como el efecto fotovoltaico, la radiación solar y la importancia de la orientación e inclinación de los paneles. Posteriormente, se presenta la memoria del proyecto, en la cual se describen el emplazamiento, el consumo energético de la vivienda y la simulación de diferentes configuraciones de potencia instalada, utilizando herramientas como PVsyst para obtener rendimientos anuales y estacionales. Asimismo, el documento detalla de forma minuciosa la composición de la instalación, abarcando desde la selección de los paneles solares (como los módulos monocristalinos de JASolar), la elección del inversor (Solax X1-HYBRID 5.0D G4) y la batería de almacenamiento (Solax Triple Power T58 5.8kWh HV Master V2.1), hasta el cableado, los soportes estructurales y las protecciones eléctricas, haciendo especial hincapié en el cumplimiento de la normativa vigente. Se exponen también los criterios de dimensionamiento de conductores, caídas de tensión y protección contra cortocircuitos, demostrando la rigurosidad técnica aplicada en el proyecto. Finalmente, el documento aborda los aspectos legales y administrativos relativos a la conexión a red y la legalización de la instalación, complementando así una visión global que integra tanto la parte técnica como la normativa y contractual. Además, de analizar económicamente el impacto económico que llega a tener la instalación en 10 años realizándose grandes ahorros en la factura para la familia. En resumen, este trabajo de fin de grado ofrece una visión completa y detallada que demuestra cómo la implementación de una instalación fotovoltaica con almacenamiento en una vivienda obtiene beneficios en la factura de la luz a lo largo de los años, resaltando la reducción de gastos energéticos en dicha vivienda.

Quantification of the H Content in Diamondlike Carbon and Polymeric Thin Films by Reflection Electron Energy Loss Spectroscopy

A nondestructive method to determine the hydrogen content at the surface of diamondlike carbon and polymeric thin films is proposed. The method relies on the analysis of the elastic peak produced by backscattering of electrons from the hydrogen atoms present at the sample surface. Quantitative analysis of the H content at the surface is achieved through use of a phenomenological sensitivity factor for elastic electron backscattering by H atoms with respect to other atoms present at the surface of reference polymeric samples. The validity of the method is checked with elastic recoil detection measurements and infrared spectroscopy analysis of the same samples. The accuracy of the method in the determination of H content at the sample surface is estimated to be ±10%.

Theory of Three-dimensional Subdiffraction Imaging

Subdiffraction image formation and measurement are analyzed. It is shown that subdiffraction imaging devices cannot produce focusing of energy into three-dimensional spots of subdiffraction size. However, three-dimensional subdiffraction imaging is possible as a consequence of a matching/tunneling process. © 2006 American Institute of Physics.

Incidence and Predictors of Worsening Renal Function in Edoxaban-Treated Atrial Fibrillation Patients Within ETNA-AF-Europe Registry

BACKGROUND Managing patients with atrial fibrillation (AF) and worsening renal function (WRF) remains a clinical challenge due to the need of dose adjustment of non-vitamin K antagonist oral anticoagulants. OBJECTIVES To determine the incidence of WRF in patients with AF treated with edoxaban, the association of WRF with clinical outcomes, and predictors of WRF and clinical outcomes in these patients. METHODS This is a subanalysis of the Edoxaban Treatment in routiNe clinical prActice for patients with non-valvular Atrial Fibrillation in Europe study (NCT02944019), an observational study of edoxaban-treated patients with AF. WRF was defined as a $25% reduction in creatinine clearance between baseline and 2 years. RESULTS Of the 9,054 patients included (69% of the total 13,133 enrolled), most did not experience WRF (90.3%) during the first 2 years of follow-up. WRF occurred in 9.7% of patients. Patients with WRF had significantly higher rates of all-cause death (3.88%/y vs 1.88%/y; P < 0.0001), cardiovascular death (2.09%/y vs 0.92%/y; P < 0.0001), and major bleeding (1.51%/y vs 0.98%/y; P ¼ 0.0463) compared with those without WRF. Rates of intracranial hemorrhage (0.18%/y vs 0.18%/y) and of any stroke/systemic embolic events were low (0.90%/y vs 0.69%/y; P ¼ 0.3161) in both subgroups. The strongest predictors of WRF were a high CHA2DS2-VASc score, high baseline creatinine clearance, low body weight, and older age. Most predictors of WRF were also predictors of clinical outcomes. CONCLUSIONS WRF occurred in approximately 10% of edoxaban-treated AF patients. Rates of death and major bleeding were significantly higher in patients with WRF than without.
