Restauración de dientes posteriores endodonciados: inlays y onlays

Abstract

La restauración del diente endodonciado tiene como meta principal el reemplazo de la estructura dentaria perdida, el refuerzo de la estructura remanente y la retención del material restaurador. OBJETIVOS: describir las técnicas de restauración con inlays y onlays, comparar los beneficios de éstos frente al uso de postes, conocer de qué factores va a depender la elección del tipo de restauración y comparar los beneficios de inlays y onlays cerámicos frente a los de composite. MATERIAL Y MÉTODO: Se han utilizado 27 publicaciones una vez aplicados los criterios de inclusión y de exclusión. RESULTADOS: La cantidad de tejido remanente es el principal factor a tener en cuenta a la hora de restaurar los dientes endodonciados. Cuando no existe estructura coronaria remanente, el uso de postes es esencial. Los inlays y onlays cerámicos transfieren menos estrés al diente que los de composites. CONCLUSIONES: La efectividad del poste es observable cuando el diente presenta una pared remanente o ninguna. Se recomienda el uso de postes de fibra en lugar de postes metálicos. Los inlays y onlays cerámicos presentan mejor adaptación marginal, mayor integridad, mejor reproducción de la anatomía y menor riesgo de descementado. Los dientes restaurados con onlays muestran mejor distribución de estrés que aquellos restaurados con inlays.

The main goal of the endodontically treated tooth’s restoration is to replace the lost tooth structure, to reinforce the remaining structure and to retain the restored material. OBJECTIVES: to describe the restoration techniques with inlays and onlays, to compare both benefits against using posts, to know of which factors depends the choice of the restoration type and to compare the benefits of using ceramic inlays and onlays with those made of composite. METHODS: After applying the inclusion and exclusion criteria, 27 articles were selected to carry out the present literature review. RESULTS: The amount of remanent dental tissue is the main point of endodontically teeth´s restoration. The use of the posts is essential when there is no remaining coronary structure. Ceramics inlays and onlays transfer less stress to the teeth structures than composites. CONCLUSIONS: The posts effectiveness is observable when the tooth shows a remaining wall or none. The use of fiber posts is advisable instead of the metallic ones. Ceramics inlays and onlays have better marginal adaptation, greater integrity, higher reproduction of the anatomy and lower risk of debonding. Teeth that have been restored with onlays show better stress distribution than those which were restored with inlays.
