La parálisis cerebral infantil (PCI) es la causa más frecuente de discapadidad motora en la edad pediátrica. La PCI es un trastorno del sistema nervioso central que se caracteriza por la afectación del sistema motor, causando defectos en el tono postural y en el movimiento. Recientemente, se ha actualizado la definición teniendo en cuenta la existencia de los trastornos asociados (como retraso mental y epilepsia). Son numerosas las causas que pueden producir la PCI siendo las más relevantes la prematuridad, el peso y la hipoxia- isquémica. Se estima que afecta a unas 120.000 personas en España. La neuropatalogía se debería fundamentalmente a una agresión en el cerebro inmaduro del bebé causada por lesiones isquémicas en diversas zonas de la corteza y el tronco encefálico dando lugar en la mayoría de los casos a un síndrome conocido como síndrome de la motoneurona superior. La PCI se puede clasificar según varios criterios. En este trabajo está centrado concretamente en uno de los tipos con mayor porcentaje de afectación, siendo este la PCI espástica . A lo largo de esta revisión bibliográfica se ofrecerá una visión global de lo que es este síndrome motor desde su origen hasta los últimos avances que conocemos en la actualidad. Presentaré el diagnóstico, una parte fundamental para un correcto tratamiento. Igualmente mostraré el tratamiento tanto farmacológico, con todos los fármacos usados actualmente, como el tratamiento no farmacológico donde tiene un papel muy importante la cirugía y por supuesto, la fisioterapia. Éste último abordaje es considerado la primera línea ante un paciente con PCI. The infant cerebral palsy (ICP) is the most frequent cause of physical disability in children. The infant cerebral palsy (ICP) is the most frequent cause of physical disability in children. ICP is a nervous system disorder characterized by motor system failures, causing defects of posture and movement. Recently, ICP definition has been updated according to existence of associated disorders (such as mental retardation and epilepsy). There are many causes that can lead to ICP, considering the most relevant prematurity, weigh, and ischemic hipoxia. ICP is estimated to affect around 120.000 people. Neuropathology is fundamentally due to an injury in the baby´s immature brain, caused by ischemic lesions in various areas of the cerebral cortex and the brain stem, leading, in most cases, to upper motor neuron syndrome. ICP can be classifed according to various criteria.Thiis work is specifically focused on one of the types with the highest percentage of involvement , this being the spactic IPC. Throughout this bibliographic review, a global vision of ICP, from its origin to nowadays lastest advances, will be discussed. Diagnosis, which is fundamental for an appropiate treatment, will be argued. Besides, we will show both pharmacological treatment, indicating all the drugs currently used, and non- pharmacological treatment, signifying the important role of surgery and, of course, physiotherapy. This last approach is considered the first-line theraphy for ICP patients. |
